Mosbach, DE

I am beginning to piece numerous things together over my 55 years, and notice that many things point to SPAD
Here's what I'm dealing with:
Breakthrough infections despite increasing prophylactic from 2x weekly azythromycin to 3x weekly doxycycline
autoimmune disease/Sjogren's
dysautonomia
long COVID

My bigger concern is the breakthrough infections. Today, my immunologist said I probably picked up a virus, as school's starting has brought about much spread of respiratory viruses

Welcome β€” it sounds like you've been on quite a journey piecing this all together, and it makes complete sense that breakthrough infections are your biggest concern right now. Your calling as a pastor puts you in some genuinely high-risk environments, so let's talk through some practical ways to help protect yourself. Since hospitals and crowded spaces are unavoidable for you, layering your protection strategies is really important. Here are some key steps that can help reduce your infection risk:

- Wear a well-fitted N95 or KN95 mask during hospital visits and in crowded indoor spaces
- Wash hands thoroughly and frequently β€” carry hand sanitizer for when soap isn't available
- Avoid touching your face, especially eyes, nose, and mouth
- Use HEPA air purifiers at home or in your office to improve air quality
- Open windows when weather allows to improve ventilation Since prophylactic antibiotics alone aren't fully controlling your breakthrough infections, it's worth having a deeper conversation with your immunologist about whether immunoglobulin (Ig) replacement therapy might be appropriate for you. For people with SPAD whose bodies can't produce enough working antibodies, Ig therapy can provide additional protection that antibiotics simply can't. Also, given your long COVID history and the fact that you're regularly in hospitals, discussing COVID-specific preventive treatments with your immunologist is worthwhile. There are options available that may offer added protection between exposures.

Your immunologist is your most important partner here β€” don't hesitate to push for a more comprehensive plan given how frequently you're exposed. You're doing meaningful work, and protecting your health is what keeps you able to do it. πŸ’™
